WYANDOTTE, Okla. —
A 9-year-old girl died of injuries Thursday afternoon on playground equipment at the elementary school in Wyandotte, Okla.
The fourth-grade student, identified as Alyssa Avila, was playing on some new playground equipment called the X-Wave with other children about 2 p.m. when she fell “and sustained a head injury as she was attempting to get back on,” according to a statement issued by the Wyandotte Nation Tribal Police.
She was taken by ambulance to Integris Baptist Regional Health Center in Miami, Okla., where she was pronounced dead at 3:55 p.m. Her body was turned over to the Oklahoma state medical examiner’s office for an autopsy.
Superintendent Troy Gray of Wyandotte schools said the equipment, which had only been in use two days, has been secured and children will no longer be able to access it. Administrators met with faculty and staff this morning prior to classes.
Counseling services are being offered to any students who need them in the wake of the accident, Gray said in a prepared statement.
